And Reaper!Dean, but that had no physical manifestation. “All” we’ve seen for Sam is actually Sam, right? I mean, in the sense that it wasn’t Sam anymore when Lucifer moved in or Meg was visiting. Sam amped up on demon juice was still Sam. It was, in fact, what Sam was meant to be. Am I missing a Sam incarnation?
